Assessment RN
Dellwood Gardens a 101-unit assisted living and memory care community in St. Paul, MN, is now hiring an experienced Assessment RN for a full-time position. Schedule includes compensated, rotating on-call status.
Dellwood Gardens is located adjacent to Swede Hollow Park and Mississippi Market. The property’s generous garden space is part of our missions to provide residents with a therapeutic natural environment in an urban setting. Fruits and vegetables grown in our gardens are prepared and served to residents at mealtimes when in season.
Qualified individuals will enjoy working with older adults and possess the ability to multitask and manage their time well, all while demonstrating highest-quality customer service centered around Dellwood Gardens’ core values of Care, Compassion, Culture and Dignity.
Primary Purpose:
This position will ensure that the health care needs of all residents are met. The Assessment RN will support Dellwood Gardens and the Clinical Nurse Supervisor in ensuring that Dellwood Gardens is in compliance with current Assisted Living with Dementia Care License regulations. The Assessment RN will work cooperatively with the Clinical Nurse Supervisor, and other nursing staff to assess new resident admits and current residents of our assisted living and memory care community.
QUALIFICATIONS:
- Must be registered and licensed with the State of Minnesota as an RN and must maintain a current and unencumbered licensure.
- Must have a valid driver’s license.
- At least 3 years similar and related experience.
PREFERENCES:
- Experience as an RN in an assisted living with memory care setting.
- Knowledge of GRH (Housing Support), Elderly Waiver, and CADI Waiver.
Other abilities and knowledge:
- Passion and aptitude for working with older adults and those with physical and behavioral health challenges.
- Leadership and supervisory skills.
- Ability to prioritize and organize tasks effectively and efficiently.
- Ability to work irregular hours, including on-call rotation (on-call on a rotating basis, which may include evenings, weekends, and holidays).
Key Responsibilities:
Complete Assisted Living with Dementia Care License regulatory assessments for incoming and current assisted living and memory care residents. Compliance and survey prep assistance and regulatory-related projects as assigned to assist with ensuring compliance; attend care conferences and meetings with case workers as needed; chart and document resident data as required and communicate to nursing staff; conduct resident assessments as necessary and appropriate; review service agreements with Clinical Nurse Supervisor; monitor and maintain client charts and nursing notes; communicate with residents’ NPs and physicians as needed; order medications; process medication renewals; fill weekly pillboxes for medication administration and check in new medications from the pharmacy; utilize staff communication book; delegate tasks to unlicensed personnel as assigned; respond to client concerns and complaints promptly and professionally.

Dellwood Gardens Assisted Living and Memory Care, a 101-unit housing with services community in St. Paul, MN, is now hiring Certified Nursing Assistants (CNAs) for full-time and part-time day, evening, and on-call positions.

REQUIREMENTS:
- 1 year experience in an assisted living setting
- 1 year experience with medication administration
- Current, active registration as a Nursing Assistant in the state of MN
PREFERENCES:
- 1 year experience as a CNA
- Experience and interest in caring for residents with mental health and behavioral challenges
Wages commensurate with experience and range from $15-$16 per hour, starting.

Housekeeper
Primary Purpose
Perform custodial and housekeeping tasks at Dellwood Gardens Assisted Living and Memory
Care. Maintain the facility in a clean, safe, and sanitary manner. Perform all tasks to assure that
the facility reflects a high quality of care and appearance.
Qualifications
• High school diploma
• Physical ability to perform manual work.
• One year prior cleaning and custodial experience.
Other Specialized Knowledge and Abilities
This position requires incumbent to have:
• Knowledge of safe cleaning chemicals that are non-allergenic.
• Ability to understand and relate well to senior adults.
• Must be flexible and able to work within a diverse team.
